This report from the Health & Safety Executive makes interesting reading regarding the safety of Octopus set ups:

To summarise its findings:
    - not all regs which comply with BS EN 250 performed adequately when two 2nd stages were being used simultaneously
    - cheaper regs or poorly maintained regs perform worse than expensive regularly serviced ones (surprise surprise).
    - in a shared ascent scenario there can be a tendency for the pair to breathe "in phase" i.e. both breathing in & out at the same time. This puts a great demand on the 1st stage. If the pair breathe "out of phase" then even the poorer performing regs can cope.
    - it is safer for the 2 sharing divers to use separate 1st stages, i.e. twin set or pony use
